  • Release forms for storage of repro materials

    Our large animal vet service currently stores reproductive materials including sperm and embryos for clients at our facility. We haven't charged clients in the past for storage costs, but are looking to create a fee because our nitrogen costs have recently increased and we are building a new facility with better storage area. Are there release forms that anyone has used in the past to advise clients of this charge as well as release the vet service from liability issues related to storage of these materials? Any advice would be appreciated.
